TITLE:Hospice and Community Chaplain

EXEMPT STATUS:Exempt – Full-Time

REPORTS TO:Clinical Supervisor, Case Manager, Executive Director

GENERAL SUMMARY:

The Hospice and Community Chaplain is responsible for the provision of spiritual care services to patients and families/caregivers of The Christian Village Hospice. Patients under the Chaplain’s spiritual care will reside or be cared for in one of Christian Village Communities two facilities. Such care will be provided directly or through coordination of care with other spiritual counselors.

The Hospice and Community Chaplain is also responsible for providing chaplain services on a part-time basis at The Christian Village at Mt. Healthy, working in accordance with established policies and procedures and/or specific instructions from the Executive Director or their designee to plan, coordinate, and direct the various spiritual needs within the Community.

PRINCIPLE DUTIES:

Essential Job Duties/Responsibilities:

Responsibilities of the Chaplain include, but are not limited to, the following:

  1. Assures spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner.
  2. Facilitates the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual needs.
  3. Provides direct spiritual care to patients and families/caregivers.
  4. Serves as liaison and support to community chaplains and spiritual counselors.
  5. Maintains records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision.
  6. Documents direct services and ongoing communication with community chaplains and spiritual counselors.
  7. Coordinates and conducts religious services and study groups for the various care levels within the community.
  8. Visits and ministers to the residents in the community and when they are in area hospitals.
  9. Coordinates special seasonal religious programs including Christmas, Thanksgiving, Memorial Services.
  10. Attends patient care conference and interdisciplinary group meetings as a member of the interdisciplinary group.
  11. Provides consultation, education, and support to the interdisciplinary group on spiritual care.
  12. Provides funeral or memorial services for patients as requested.
  13. Oversees the bereavement program.
  14. Plans periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team.
  15. Assists in supervision of spiritual care volunteers when assigned to patients/ families/caregivers.
  16. Actively participates in quality assessment performance improvement teams and activities.
  17. Accepts and performs other related duties and responsibilities as required.
  18. On call/ after-hours services
  19. Prepares reports as requested.

Position Qualifications:

  1. Graduate of accredited seminary or school of theology (BA, MA, BD, MDiv or equivalent theological degree).  Experience working with death and dying, individuals/family/caregiver.
  2. Ability to work as a member of interdisciplinary group.
  3. At least one unit of Clinical Pastoral Education (CPE) is preferred.
  4. Comfort in an interfaith setting.
  5. Ability to accept people of different cultures, beliefs, and values.
  6. Ability to network with community clergy and congregations.
  7. Knowledge of and commitment to hospice philosophy of care.
  8. Experience in grief and loss counseling.
  9. Hospice experience preferred.
  10. Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.

WORKING CONDITIONS:

  3. In and out of an automobile.

PHYSICAL REQUIREMENTS/WORKING CONDITIONS:

Ability to move throughout the facility promptly, especially in emergency situations. Must be able to lift, unassisted, up to 50 lbs. and up to 150 lbs. with the assistance of one. Must be able to stoop, bend, and reach. Must be able to push and pull up to 150 lbs. in a wheelchair.  Good visual and hearing acuity required. Good verbal communication skills required. Must be able to walk or stand for up to 80% of the workday. Must be free of communicable disease. May be exposed to communicable diseases and blood and body fluids that may contain HIV and/or HBV. May be exposed to cleaning chemicals and offensive odors.
